Supplemental Forms
Generally, most students are required only to complete the Free Application for Federal Student Aid (FAFSA), and, if interested in applying for loans, a Master Promissory Note.
From time to time, you may be asked to complete additional financial aid forms.
You can review the "Documents needed to complete your application" from your portal page at ERAU Online to determine if you are required to complete any of the forms listed below.
If so, you can obtain the forms from our offices or conveniently download and print the forms from this page. Return the completed forms to the Financial Aid Office of the campus you're attending. (For fax numbers and addresses, click here.)

Return of Title IV Funds Financial Aid Programs Policy and Standards of Satisfactory Academic Progress for Financial Aid Applicants
This brochure's first section contains information for students who receive financial aid and withdraw from all classes within a term.
