Supplemental Forms

Generally, most students are required only to complete the Free Application for Federal Student Aid (FAFSA), and, if interested in applying for loans, a Master Promissory Note. From time to time, you may be asked to complete additional financial aid forms. To determine if you are required to complete additional forms, login to ERNIE and access Financial Aid: Information under Colleague Student Services.

Return the completed forms to the Financial Aid Office.

Return of Title IV Funds Financial Aid Programs Policy and Standards of Satisfactory Academic Progress for Financial Aid Applicants
This brochure's first section contains information for students who receive financial aid and withdraw from all classes within a term. The second section details the satisfactory academic progress that financial aid recipients must maintain to continue receiving financial aid.

If you intend to use federal loans, you must complete the Entrance Loan Counseling Form and the Master Promissory Note. Login to ERNIE and access Financial Aid: Apply for Stafford Loan under Colleague Student Services.
