Medical Report Form

This form is mailed by the Department of Admissions as students are confirmed for admission to the university.

It must be completed and returned to Health Services, prior to enrollment, in the addressed envelope provided.

Please note:
Certain immunizations are required for continued enrollment and / or
residence in university managed housing.
